All records requests shall be in writing and signed by the requestor.

Requests will be accepted from residents of the Commonwealth of Kentucky as defined in KRS 61.870.

All records requests must specifically state the documents/records to be inspected. Be specific about what you need. Include names, dates, keywords, or other details that can help expedite the processing of your request.
